504 Plan Renewal Process

Requirements for Annual 504 Plans
  1. One month prior to the annual review date, the school counselor will schedule a meeting with the family, the assigned administrator, and student's teachers.
    • A mutually agreed upon date will be established and it is the expectation that all 504 Plans will be reviewed annually (within one month of the previous year’s meeting date).
    • Team members must be present or participating via conference call in all 504 meetings.
    • Signatures are required on 504 Plans. Emails are not permitted as agreement. Team members required to sign for a review of a 504 Plan are the school counselor, teacher, and district administrator.
  2. Changes to 504 Plans - it is a requirement to conduct an Evaluation under Section 504 prior to making any change in placement.
Requirements for Reevaluation Under Section 504
  1. The team will conduct a 504 evaluation at least every three years. This requires opening a new 504 evaluation template in Samegoal.
  2. The school counselor will gather multiple sources of recent data which can but are not limited to include: teacher feedback, academic data, medical updates, and disciplinary records.
  3. The school counselor will schedule meeting and invite team members which include the Parent, Administrator and the student's Teachers. The school counselor should also consider inviting any team member who is knowledgeable about the child, evaluation data, or placement options and accommodations. This can include the student.
  4. The team will review data and determine if continued eligibility is warranted. If yes, discuss which current accommodations are effective, which are no longer needed, and if any new accommodations are required based on the student's current grade level and environment.
  5. The school counselor will create a new 504 Plan in Samegoal to reflect the evaluation of accommodations and obtain signatures from team members. A parent signature is not required during a reevaluation, but it is district practice to document the attempts to gain a parent signature.
